Faraja Trust Fund is a registered non-governmental organization with aims to empower communities among them women, men and the most vulnerable groups, to address development challenges through a participatory approach and transfer of skills and knowledge for sustainability. Currently Faraja Trust Fund is seeking to recruit a Community Based HIV Services Focal Person for USAID Boresha Afya Program.
Job Title: Community Based HIV Services Focal Person
Reporting to: Project Coordinator
Work Station: Morogoro District Council
Vacant post: 3 posts
Minimum Qualification
a) Holder of Diploma in Nursing, Social Work, Counseling, Community Development or Social Science
b) Must have computer skills
c) Effective written and oral communication in English and Swahili languages
d) Experience of not less than two years in project management
e) Experience working with donor funded projects (added advantage)
Duties and Responsibilities
i. Support and assist CBHS project coordinator in the implementation of integrated CBHS activities
ii. Supervise CBHSP in provision of comprehensive integrated care in the community
iii. Facilitate bidirectional linkage of PLHIV
iv. Facilitate tracking of clients who miss their appointments or lost to follow up
v. Monitor and evaluate CBHS activities
vi. Prepare monthly/quarterly/annual reports
vii. Supervise data collection and reports from volunteers
viii. Perform any other relevant duties as assigned by the management.
